Health Clearance for Your Programme University of Greenwich

If you have applied to study on a programme at the University of Greenwich that requires health clearance, you will need to complete a confidential health questionnaire.

The process is slightly different depending on whether you are studying an Education or Health programme.

If you are studying on an Education programme

  1. You will receive notification via email that you should complete an online confidential health questionnaire once you have accepted the offer of a place on your programme
  2. Please follow the link in the email to access the GP Declaration Form. Print this form and arrange for your GP to complete and sign it, as you will be required to upload the form when submitting your online occupational health questionaire.

If you are studying on a Health programme

  1. Once you have accepted an offer of a place on your programme, you will be posted a health clearance application by the Student Centre*
  2. The health questionnaire should be completed and handed to your GP to countersign the declaration form.
  3. The completed form should then be sent to the Occupational Health department as advised in the Joining Instructions.
  4. You should then telephone the appropriate Occupational Health department to arrange a suitable time to attend for medical screening.
