Health Clearance for Your Programme

If you have applied to study on a programme at the University of Greenwich that requires health clearance, you will need complete a confidential health questionnaire.

Receiving your health questionnaire

Once you have accepted an offer of a place on your programme, you will be posted a health clearance application by the Student Centre, together with your CRB Disclosure application.

What to do with your application

The health questionnaire should be completed and handed to your GP.  Once the GP has signed the Declaration form, the following process should be followed:

  1. Teaching, Youth Work, Early Years students

    Please return the (white) questionnaire to the Avery Hill Student Centre.  If our Medical Officer requires any further information you will be contacted directly.

  2. Social Work students

    The completed (green) form should be returned to the Occupational Health Department as advised in the enclosed letter. The Occupational Health: Return Confirmation Form should be sent to the Student Centre, Southwood House, Avery Hill Campus, London SE9 2UG.

  3. Nursing / Midwifery students

    The completed form should be sent to the Occupational Health Department as advised in the accompanying Joining Instructions. You should then telephone the Department to arrange a suitable time to attend for medical screening.

Fees

For School of Education students only, there is a fee of £10 for the health clearance application, that is collected at the same time as the CRB fee of £40, meaning that for both forms there will be a total fee of £50. Cheques should be made payable to the University of Greenwich. Cash or card payments can be made in person in the Student Centre during normal working hours (postal orders cannot be accepted).

Please note that we are unable to refund the fee if, for any reason, you do not join the programme in September. In view of this, you may wish to wait until you have satisfied any conditions of the offer before submitting the application.

Training Bursary (for one-year PGCE students only)

UK and EU fee-paying students fully-registered on one-year PGCE programmes may be eligible to receive the Government training bursary.  This is paid in 9 monthly payments directly into your bank account. The form can be submitted to the Student Centre at any time and payments will commence when you are fully-registered as a student of the University. To be fully-registered you will need to have shown all relevant qualifications and submitted a satisfactorily-completed CRB application form and health questionnaire.
